Credentialing Steps

  • Step 1- Ensure that the provider is already approved through NC Tracks

  • Step 2- Fill out the Enrollment Initiation Form: LP and LIP

  • Step 3- Send it via email to: Credentialing Team @ Vaya

  • Step 4 ******Add the documents sent via email to the Monday Credentialing Board and document updates completed.

    • Add a follow up date on the Board for 30 days and continue to follow up using contact information below!
