Step 1- Fill out Certification Application Request & Submit
Add a due date to the Monday Board x 30 days and make notes on work completed
Step 2- Fill out any documents sent over by the certification department & submit
Add information and documents to Monday Board
Step 3- Create online account (Provider Portal) on behalf of the provider
Add the log in information to the Monday Board
