Credentialing Steps
Step 1- go to Provider Express Website and Log in using these credentials”
Provider Express User Id- Mypeakpractice27502 PW- Pps27502
Step 2- Click Link New Provider (upper right hand corner)
Step 3- Complete the application and submit
You will need to add the W9 here.
Step 4- Add a screenshot of the application ID submission to the Monday Credentialing Board and document updates completed.
Add a follow up date on the Board for 30 days and continue to follow up using contact information below!
